Innovative Care accepts most major insurance plans (PPOs,
Medicare and the Advocate hospital system HMO).

SELF-PAY PRICING
No insurance necessary. We will tell you the
cost of your treatment up front so you have full
control before, during and after your visit.

Basic Visit | $175
This is a basic visit for possible flu, strep, UTI or a simple workup or treatment for fever. This visit involves one test (rapid flu, rapid strep, or urine testing), but does not involve blood testing or X-ray.
Annual Physical | $250
This is a general check-up that includes a variety of labs such as your complete blood count, chemistry panel, thyroid check, cholesterol panel and glucose level. The annual physical does not include STD testing* or a women’s health gynecologic exam.

Telemedicine | $100
Allows you to see a provider virtually. Many medical issues can be addressed via telemedicine. Telemedicine is also commonly used for medication refills (Adderall, Vyvanse, Xanax).
Follow-Up Vist $100
This is a follow up appointment for a condition in which you have already been seen (within a month) or for a refill on a medication. This does not include blood work, which would be billed at the complex visit fee.
Complex Visit | $250
This visit is for those who are injured or sick and experiencing symptoms that will require blood work or an X-ray to make a diagnosis. A complex visit is also billed for laceration repair if uninsured. Please note, this price does not include crutches, boot, splints or prescription medications.
Women’s Health | $350 Physical
This general check-up and well visit includes your annual physical, as well as a gynecological exam (pap smear, STD screening).
STD Testing | Starting at $150
We offer STD screening and the full range of STD testing. Choose from one of three plans based on your individual needs
Prescription Medication | $15
If we have the medication that we prescribe for you in stock, we will charge you a fraction of what you’d pay at a pharmacy. Most medications are billed $15 for first medication and $10 for additional medications compared to self-pay Z-pak at the pharmacy will be more than $100.
Labs | Variable Cost
There are some conditions that need an outside lab to process more complex lab tests, such as STD testing. We send some of these tests to our outside lab, and they will bill you for this separately. We only use the highest quality and most cost-effective labs in the Chicagoland area.
