We will provide you with the information needed to complete your application online after your appointments are complete. A temporary card is typically issued by the Illinois Department of Public Health (IDPH) within 24 hours of submitting your application.

If you do not have insurance, or simply wish to self-pay for your visits, our fee is $175. Medicaid patients and veterans pay a reduced rate of $100 per visit.
There is a $50 application fee per year, or $125 for 3 years. If you are a veteran, a senior or have a SSI/SSA disability, the fee is reduced to $25 per year.

Applying for your Medical Card in Illinois is now easier and faster than ever. Most patients are able to complete their telemedicine visits with us within one week. Once you complete the required telemedicine visits with our doctors, we will provide you with the Physician Certification. You will then submit your IDPH application online, and the state will provide a temporary card that you can start using within 24 hours.
