You will need to have a PTSD evaluation with one of our Innovative Therapy therapists and will be contacted to schedule this appointment. Insurance is accepted.

Insurance Accepted
Innovative Care accepts insurance for PTSD evaluations and the medical card certification appointments. A complete list of insurance plans we accept can be found here. If you choose to self-pay, the fee is $175.
Medicaid patients: Innovative Wellness cannot bill medicaid for your appointments, but we do offer a reduced self-pay rate of $100. Please bring your Medicaid insurance card to receive the reduced rate.
Veterans: Veterans who are self-pay are also able to take advantage of the reduced self-pay rate of $100 per visit. Proof of veteran status will be required to be eligible for the reduced rate.

Therapy Evaluations: During your appointment, we will assess the severity of your symptoms to make a formal PTSD diagnosis. By definition, people who develop PTSD have experienced some sort of trauma in the past that creates significant anxiety, depression, sleep disturbances, or other mental health issues that interfere with their ability to function. If this description does not pertain to you, then you may not want to pursue an evaluation. Not everyone who experiences a traumatic event develops PTSD. If you DO feel this describes you, please get evaluated.
Note, these therapy appointments are specifically to evaluate patients for PTSD for the purpose of obtaining an Illinois Medical Card.
We require medical documentation noting that you have PTSD (it must say PTSD on the diagnosis). If you have this, you may begin your Innovative Wellness appointments for the Illinois Medical Card. If you do not have a confirmed diagnosis, you must first have a therapy evaluation.
PTSD evaluations take approximately 30 minutes via telemedicine.
